Retatrutide: The Next Big Thing?
Retatrutide (LY3437943) is a first-in-class triple-receptor agonist developed by Eli Lilly. It targets three metabolic hormone receptors simultaneously โ a mechanism no approved drug has achieved before. With Phase 3 data showing 28.7% average body weight reduction, it has produced the highest weight loss efficacy ever recorded in clinical trials.
The Three Receptors
GLP-1
Suppresses appetite, delays gastric emptying, enhances glucose-dependent insulin secretion
GIP
Enhances insulin sensitivity, improves fat tissue metabolism, amplifies GLP-1 effects
Glucagon
Increases energy expenditure, promotes lipolysis (fat breakdown), enhances hepatic fatty acid oxidation
The glucagon receptor is what sets retatrutide apart. While semaglutide (GLP-1 only) and tirzepatide (GLP-1 + GIP) primarily reduce caloric intake, retatrutide also increases energy expenditure โ attacking the energy balance equation from both sides simultaneously.
Clinical Trial Results (TRIUMPH Program)
TRIUMPH-4 (December 2025)
28.7% average weight loss at 68 weeks in adults with obesity and knee osteoarthritis. Significant improvements in pain and physical function. Both 9mg and 12mg doses met all primary and secondary endpoints.
TRANSCEND-T2D-1 (March 2026)
Up to 2% A1C reduction and 16.8% weight loss in just 40 weeks in adults with type 2 diabetes. No weight loss plateau observed โ participants were still losing weight when the trial ended.
What's Next?
Seven additional Phase 3 trials are expected to report results throughout 2026, covering obesity without diabetes, type 2 diabetes with cardiovascular disease, and other metabolic conditions. If all trials succeed, Eli Lilly is expected to submit for FDA approval in late 2026 or early 2027, with potential commercial availability by mid-to-late 2027.
