The GLP-1 Showdown: Which Stands Out?
The metabolic peptide landscape has evolved rapidly. Three incretin-based compounds now dominate the research space, each targeting different combinations of hormone receptors. Understanding the differences is essential for any researcher working with metabolic peptides.
The Three Generations
Semaglutide
Single agonist — GLP-1 only
Average weight reduction in Phase 3 trials. Active compound in Ozempic and Wegovy.
Tirzepatide
Dual agonist — GIP + GLP-1
Average weight reduction. Active compound in Mounjaro and Zepbound.
Retatrutide
Triple agonist — GIP + GLP-1 + Glucagon
Highest efficacy ever recorded in clinical trials to date.
Why Does the Third Receptor Matter?
Retatrutide's addition of the glucagon receptor is what researchers believe drives the significantly greater efficacy. Glucagon increases energy expenditure, promotes lipolysis (fat breakdown), and enhances fatty acid oxidation in the liver. Combined with GLP-1's appetite suppression and GIP's insulin-sensitising effects, the triple mechanism creates a more comprehensive metabolic intervention than single or dual agonists alone.
Where Does the Research Stand in 2026?
The Phase 3 TRIUMPH program is the largest clinical trial program ever conducted for a weight loss compound. The first results from TRIUMPH-4 showed 28.7% average body weight reduction at 68 weeks. The TRANSCEND-T2D-1 trial showed up to 2% A1C reduction and 16.8% weight loss in just 40 weeks — with no plateau observed, meaning participants were still losing weight when the trial ended.
Seven additional Phase 3 trials are expected to report throughout 2026. If successful, Eli Lilly is expected to submit for FDA approval in late 2026 or early 2027.
